Instructions for filling in the Occupational Health and Safety Performance Self-Assessment Scale in the corporate sustainability report (applicable to Financial and Insurance Enterprises)

I. Purpose and Basis
To guide enterprises in continuously improving their Occupational Health and Safety (OHS) performance and aligning with global ESG sustainable development trends, the Occupational Safety and Health Administration (OSHA), Ministry of Labor, has established this industry-specific performance assessment platform. This assessment provides a standardized, quantifiable benchmark to help financial and insurance enterprises self-evaluate their OHS achievements. It encourages companies to enhance management performance and transparency year by year, and proactively disclose their execution results in corporate sustainability reports, annual reports, or official websites, concretely fulfilling their sustainable commitment to workplace health and safety.

II. Target Audience
This assessment is specifically designed for the financial and insurance industry chain, primarily targeting enterprises that have compiled a corporate sustainability report. (It is recommended to prioritize a comprehensive assessment of OHS performance for the entire group at the enterprise level; enterprises may also assign different subsidiaries or branches to conduct independent assessments based on practical execution and division of labor):

  • Listed Companies: Enterprises whose industry category is classified as "Financial and Insurance" or "Financial" according to the industry classification guidelines of the Taiwan Stock Exchange (TWSE) or the Taipei Exchange (TPEx).

  • Non-Listed Companies: Enterprises engaged in financial services, insurance, securities and futures, and financial auxiliary activities according to the Standard Industrial Classification (Revision 11) by the Directorate-General of Budget, Accounting and Statistics (DGBAS).

  • (Note: Enterprises or business units that have not yet compiled a sustainability report can also use this assessment to review their internal OHS implementation; however, some indicators involving "public disclosure" may not be applicable without external public documents or channels.)

III. Assessment Structure and Official Disclaimer

  1. Design Basis and Dimensions: Based on OSHA's 2024 Practical Guidelines for SDG Disclosure on Workplace Sustainable Health and Safety, this assessment integrates 10 criteria into 8 major dimensions for scoring. These include: Systematic Management, Risk Assessment and Management, Worker Participation, Education and Training, Health Services and Promotion, Supply/Value Chain Impact Management, and Occupational Injuries and Work-Related Diseases.

  2. Industry-Specific Indicators and Scoring:

    • Implementation Assessment: Tailored to the characteristics of the financial and insurance industry and expert recommendations, specific performance indicators are established by extracting leading indicators from the Guidelines, incorporating corporate commitments and self-declarations (e.g., evaluating whether the enterprise publicly commits to making OHS a material issue and sets quantitative management goals). Enterprises should select the latest publicly available sustainability report as the assessment basis (e.g., assessing 2023 performance in 2024) and answer using a "single-choice" format for the completeness of execution for each indicator.

    • Bonus Indicators: Based on industry characteristics and focus areas, enterprises are expected to publicly disclose more valuable advanced indicators. Please use a "multiple-choice" format to select the descriptions that match your actual execution.

  3. Official Disclaimer: This assessment aims to promote corporate self-management and focus on key performance evaluations. Leading indicators not included do not mean the enterprise does not need to execute them; the assessment results do not represent regulatory compliance or serve as any form of legal certification.

IV. Guidelines and Platform Notices

  1. Pre-Assessment Review: A "Complete Assessment PDF" and "Filling Instructions" are attached at the bottom of this webpage. It is highly recommended to download them first, convene relevant internal departments for review, confirm data and supporting information, and then complete the online form in a single session.

  2. Result Certificate and System Storage: Once the online form is completed and submitted, the system will instantly generate a self-assessment result report. Since the platform does not retain historical records, please be sure to download and print the certificate report for your records before closing the browser window.

  3. Regular Review Mechanism: This assessment supports repeated submissions, and the system will solely recognize the data from the last successful submission. Enterprises are welcome to fill it out regularly as an internal governance tool to track OHS performance growth.

  4. Supply Chain Referral Code Mechanism:

    • If your company is a supply chain partner invited to fill out the form, please enter the inviting enterprise's 4-digit stock code or 8-digit tax ID in the "Supply Chain Referral Code" field.

    • If invited by multiple enterprises simultaneously, please enter all of them and make sure to separate them with a semicolon (;).

    • Example: If invited by Company A (Stock Code 3333) and Company B (Tax ID 34444666), you should enter: 3333; 34444666.

  5. Information Security and Privacy Statement: Individual enterprise assessment data and score results are strictly confidential. The platform will never disclose this information to the public, nor will it be used by the public sector as a basis for increasing labor inspections or audit frequencies.
