I. Purpose and Basis
To guide enterprises in continuously improving their Occupational Health and Safety (OHS) performance and aligning with global ESG trends, OSHA has established this self-assessment platform based on the "Workplace Health and Safety Sustainability Development Project." This assessment provides a standardized, quantifiable benchmark to help enterprises evaluate their OHS achievements, serving as an effective communication tool for internal strategic refinement, sustainability information disclosure, and supply chain management.
 

II. Target Audience
This assessment applies to all industry categories EXCEPT the specified industries (Electronics, Financial & Insurance, Construction, Chemical, Transportation, and Steel):

• Listed Companies: Please confirm your industry category based on the industry classification guidelines of the Taiwan Stock Exchange (TWSE) or the Taipei Exchange (TPEx).

• Non-Listed Companies: Please refer to the Standard Industrial Classification (Revision 11) by the Directorate-General of Budget, Accounting and Statistics (DGBAS). If no specific industry assessment applies, please use this Generic Version.
   

III. Assessment Structure and Official Disclaimer

1. Design Basis: This assessment is systematically designed based on the 2024 revised Practical Guidelines for SDG Disclosure on Workplace Sustainable Health and Safety by OSHA.

2. Assessment Dimensions and Weighting: The assessment consists of 45 indicators and 1 corporate commitment declaration, divided into three main themes:

   • A. GRI 403 Basic Indicators: 20 indicators (60% weight, True/False format) to review the organization's foundational management baseline.

   • B. GRI 403 Advanced Indicators: 25 indicators (25% weight, True/False format) to evaluate the maturity of the management mechanism.

   • C. OHS Commitment and Performance Self-Declaration: 1 open-ended question (15% weight) guiding enterprises to specifically describe improvement goals, practical feasibility, and measurement methods.

3. Official Disclaimer: This assessment aims to promote corporate self-management and focus on key performance evaluations. Indicators not included do not exempt enterprises from their statutory obligations. Furthermore, the assessment results do not represent regulatory compliance or serve as any form of legal certification.
    

IV. Guidelines and Platform Notices

1. Pre-Assessment Review: A complete assessment PDF file and filling instructions are attached at the bottom of this webpage. We strongly recommend downloading them first, convening relevant internal departments for review, confirming the data and supporting information, and then completing the online form in a single session.

2. Result Certificate and System Storage: Once the online form is completed and submitted, the system will instantly generate a self-assessment result report. Since the platform does not retain historical records, please be sure to download and print the certificate report for your records before closing the browser window.

3. Regular Review Mechanism: This assessment supports repeated submissions, and the system will solely recognize the data from the last successful submission. Enterprises are welcome to fill it out regularly as an internal governance tool to track OHS performance growth.

4. Supply Chain Referral Code Mechanism:

   • If your company is a supply chain partner invited by a client or central factory, please enter the inviting enterprise's 4-digit stock code or 8-digit tax ID in the "Supply Chain Referral Code" (供應鏈推薦碼) field.

   • If invited by multiple enterprises simultaneously, please enter all of them and make sure to separate them with a semicolon (;).

   • Example: If invited by Company A (Stock Code 3333) and Company B (Tax ID 34444666), you should enter: 3333; 34444666.

5. Information Security and Privacy Statement: Individual enterprise assessment data and score results are strictly confidential. The platform will never disclose this information to the public, nor will it be used by the public sector as a basis for increasing labor inspections or audit frequencies.
